7In the current story, no, because Makuta can only shapeshift when he is in his own armor. He's not in his armor, he's in the body of Maxilos, so he can't shapeshift at all right now.

1Don't forget that Makuta also have the Adaptation power in addition to shapeshifting, so they could get rid of the effects of the mutagen whenever they wanted, even though they would be affected by it.

2When you go though trauma, you forget something at times. He just knew that there was his team "standing" in front of him, about to die, and he knew he could save them. He obiously ignored the facts and believed what he saw. If you lost someone, and you were tricked into thinking you got a second chance to prevent what had happened, wouldn't you? Sorry for the long, kinda repetitive post.

2Perhaps he was so happy to be able to do something and see his team again, he didn't want to notice it, and blocked it out.
3I think it could be an attribute of the Olisi. It could block the victims memory of the true event while the mask is in use, giving the visions Karzahni hands out more impact. That would be the nagging doubt at the back of Lesovikk's mind, his temporarily repressed memories.
